CAMP AGREEMENT
In registering my child/children for camps with ART LLC, DBA Ridgway Creative Arts, I acknowledge that certain camps have prerequisites and expectations that have been defined on the camp website. I believe my child/children meet(s) these requirements and it is my responsibility to support them in meeting the expectations.
If leadership of ART LLC, DBA Ridgway Creative Arts determines that my child/children are not meeting the requirements and/or expectations, the child/children may be asked to leave the camp. A full refund would be provided if my child/children is/are asked to leave the camp for this reason.
I acknowledge the following Cancellation Policy:
– Full refund if registration is cancelled up to 2 weeks prior to the camp start date
– 50% refund if registration is cancelled within 2 weeks prior to camp start date, up until the day before camp start date
– No refund after camp has started
ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY
I understand that I (my child/children) will be participating in a music/creativity program through ART LLC, DBA Ridgway Creative Arts that will require physical exertion through games, play breaks, etc. The camp participation may also include the use of equipment and/or musical instruments. I recognize that there is a risk of serious injury (and in extreme cases, death) associated with any movement program and/or use of equipment.
I understand that my child/children may store personal equipment at the camp location. While a genuine effort will be made to store this equipment securely, I understand there is risk of personal equipment being damaged and/or stolen.
By submitting this form, I am agreeing to NOT hold ART LLC, DBA Ridgway Creative Arts or any of its employees, owners, camp volunteers, agents or insurers responsible for any bodily injury or property damage that I (my child/children) may suffer as a result of participating in music/creativity camps with ART LLC, DBA Ridgway Creative Arts.
